# Intravenous ipamorelin in people

> The human return | Route 66 Peptides — Route 66 Peptides returns from oral analogues to intravenous ipamorelin: healthy male volunteers, a growth-hormone pulse and bowel-surgery trial results.

The oral detour leads back to a smaller human record on the parent molecule, delivered by a different route.

## Healthy volunteers receiving intravenous ipamorelin

The [human pharmacokinetic study](https://pubmed.ncbi.nlm.nih.gov/10496658/) characterized intravenous ipamorelin in healthy male volunteers. Its terminal half-life was approximately 2 hours. The growth-hormone response took the form of a single discrete pulse, peaking at about 40 minutes after dosing.

Those clocks deserve separate attention, I think. One describes the decline in the peptide's plasma concentration; the other locates the peak of the hormone response. Neither clock measures muscle gain, sleep quality or the practical recovery of someone after surgery. They characterize a particular exposure and its hormone response.

> These measurements come from intravenous administration of the parent peptide in healthy men.

## Median meal-tolerance times

The [randomized proof-of-concept trial](https://pubmed.ncbi.nlm.nih.gov/25331030/) involved 114 adults undergoing open or laparoscopic bowel resection. Median time to the first tolerated meal was 25.3 hours with ipamorelin and 32.6 hours with placebo; p=0.15. The trial missed its primary endpoint.

The direction of the medians favoured ipamorelin. For me, those medians offer weaker encouragement than a successful endpoint test would have. A missed endpoint leaves room for uncertainty about the size of an effect. More persuasive clinical results would be necessary before I described the compound as a useful way to restore eating after bowel surgery.

## The short safety window

In that [perioperative trial](https://pubmed.ncbi.nlm.nih.gov/25331030/), treatment-emergent adverse events occurred in 87.5% of the ipamorelin arm and 94.8% of the placebo arm. The report identified no ipamorelin-specific safety signal in the short perioperative window. Those percentages describe events appearing during treatment; they do not assign every event to the drug.

The [broader peptide-therapy review](https://pubmed.ncbi.nlm.nih.gov/41966639/) emphasizes scarce rigorous human safety data. For confidence to grow, I need evidence for the population, route and duration being proposed. A brief surgical trial and an acute volunteer experiment give a limited kind of reassurance.

## A useful return journey

The [oral analogue experiments in dogs](https://pubmed.ncbi.nlm.nih.gov/9733496/) answer a chemical-development question. The [intravenous human study](https://pubmed.ncbi.nlm.nih.gov/10496658/) answers a question about exposure and hormone release. The [bowel-surgery trial](https://pubmed.ncbi.nlm.nih.gov/25331030/) asks about recovery of eating. This untidy itinerary suits me; each stop has a particular purpose.

The [reading list](/references) brings those destinations together. The research becomes more interesting when the destination has a name.
